Prepare for the preseason by ensuring all athletes complete the required paperwork for athletic clearance. Download your physical clearance packet using the link provided below. Complete the packet, have your physical examination done, and get it signed by your doctor. This process is managed through an app called AKTIVATE, available for download in your app store. Make sure to include the sports you wish to participate in. Finish your annual physical, secure the doctor's signature, have the documents notarized, and upload them to your AKTIVATE account.
